The Social Services Assistant is responsible for ensuring that residents' social and emotional needs are met in a caring and professional manner. Working in alignment with federal, state, and local regulations, this role plays a critical part in promoting residents' well-being in collaboration with the healthcare team. The ideal candidate has experience in social services within a healthcare or skilled nursing facility setting and is passionate about providing individualized care.
Duties and Responsibilities:
Resident Support and Advocacy:
- Assess and address the emotional, social, and mental health needs of residents, ensuring individualized care.
- Develop and maintain positive relationships with residents, fostering a supportive and compassionate environment.
Care Planning and Documentation:
- Contribute to resident care plans by documenting observations, assessments, and interventions.
- Collaborate with nursing staff and interdisciplinary teams to integrate social services into residents' overall care plans.
Compliance and Best Practices:
- Ensure compliance with federal, state, and local social services regulations and guidelines.
- Adhere to and uphold The Selfhelp Home's standards of care, aligned with Medicare and The Joint Commission requirements.
Family and Community Engagement:
- Support families in understanding resident needs, offering resources and information about available social services.
- Assist in coordinating family meetings to discuss care plans and address concerns.
Program Development and Activities:
- Assist in planning and implementing social and recreational activities to enhance residents' quality of life.
- Actively participate in creating an inclusive environment that promotes the cultural and social needs of residents.
Other Duties as Assigned:
- Complete additional tasks to support resident well-being and the mission of The Selfhelp Home as directed by the Director of Social Services.
Qualifications:
Education and Experience:
- Education: Associate's degree in Human Services (e.g., Social Work, Psychology, Sociology).
- Experience: 2 years of social services experience, preferably in a healthcare setting. Skilled nursing facility experience is highly desirable.
Skills and Abilities:
- Excellent interpersonal and communication skills, with a compassionate approach to resident care.
- Ability to work effectively as part of an interdisciplinary healthcare team.
- Strong organizational skills, with a high level of attention to detail.
- Knowledge of social service regulations and best practices within a skilled nursing facility setting.
