The Case⠀Manager⠀will be⠀responsible⠀for⠀providing⠀comprehensive⠀case⠀management⠀services⠀to⠀individuals⠀or⠀families⠀in need.⠀The Case⠀Manager⠀will⠀assess⠀client⠀needs,⠀develop⠀and⠀implement⠀a service⠀plan, and⠀monitor⠀progress⠀toward⠀goals. The⠀Case⠀Manager⠀will work⠀collaboratively⠀with other⠀service⠀providers⠀and⠀community⠀resources⠀to ensure⠀that⠀clients⠀receive⠀appropriate⠀services⠀and⠀support.
Responsibilities:
- Conduct⠀initial⠀client⠀assessments⠀to⠀identify⠀needs and⠀develop a⠀service⠀plan
- Provide⠀ongoing⠀case⠀management⠀services,⠀including⠀regular⠀check-ins⠀and⠀follow-up
- Monitor⠀progress⠀toward⠀goals and⠀adjust the⠀service⠀plan as⠀needed
- Collaborate⠀with other⠀service⠀providers⠀and⠀community⠀resources⠀to⠀coordinate⠀services⠀and⠀support
- Advocate⠀for⠀clients⠀and⠀connect⠀them with⠀appropriate⠀resources⠀and⠀services
- Maintain⠀accurate⠀and⠀up-to-date⠀case files⠀and⠀documentation
- Provide⠀crisis⠀intervention⠀and⠀support as⠀needed
- Participate⠀in team⠀meetings⠀and case⠀conferences
- Keep⠀up-to-date⠀with⠀industry⠀standards⠀and best⠀practices⠀for case⠀management
Requirements:
- Bachelor's⠀degree in⠀social⠀work,⠀psychology,⠀or related⠀field
- 4-5+⠀years of⠀experience⠀in case⠀management⠀or related⠀field
- Knowledge⠀of social⠀services⠀and⠀community⠀resources
- Strong⠀interpersonal⠀and⠀communication⠀skills
- Ability⠀to work⠀independently⠀and as⠀part of a⠀team
- Excellent⠀organizational⠀and time⠀management⠀skills
- Strong⠀problem-solving⠀and⠀critical-thinking⠀skills
- Ability⠀to⠀maintain⠀confidentiality⠀and⠀professional⠀boundaries
- Experience⠀working⠀with⠀diverse⠀populations⠀and⠀cultural⠀sensitivity
- Valid⠀driver's⠀license⠀and⠀reliable⠀transportation
