Job Description

• Manage and⠀oversee a⠀team of⠀community⠀health⠀workers⠀who will⠀be⠀responsible⠀for⠀identifying⠀and⠀addressing⠀SDOH needs⠀in the⠀community,⠀as well as⠀connecting⠀patients⠀with⠀physicians⠀for⠀clinical⠀care,
• The⠀team of⠀Community⠀Health⠀Workers⠀will cover⠀all our 12⠀Hospitals⠀across 8⠀counties,⠀FQHC⠀Partners,⠀Community⠀Physicians,⠀etc,
• Develop⠀and⠀implement⠀effective⠀strategies⠀to ensure⠀that the⠀team is⠀meeting⠀performance⠀goals and⠀providing⠀high-quality⠀care to⠀patients.
• Provide⠀direction⠀for⠀community-based⠀services⠀and⠀partnership⠀initiatives⠀with other⠀Hospital⠀and⠀ambulatory⠀departments,⠀Population⠀Health,⠀Strategy,⠀and core⠀leadership
• Will⠀also⠀develop⠀relationships⠀with⠀community⠀partners⠀and⠀leaders to⠀support⠀growth,⠀reputation,⠀awareness,⠀and⠀community⠀health⠀priorities.
• Interacts⠀with⠀leadership⠀across the⠀RWJBarnabas⠀Health⠀System⠀market,⠀including⠀providers,⠀to build⠀support⠀for and⠀increase⠀involvement⠀in the⠀enterprise's⠀community⠀and social⠀impact⠀agenda.
• Identifies⠀key⠀corporations⠀and⠀businesses⠀in each⠀region and⠀oversees⠀the⠀development⠀and⠀execution⠀of plans⠀to build⠀relationships⠀with⠀them.
• Be⠀the key⠀leader to⠀establish⠀partnership⠀with all⠀our 12⠀Hospitals⠀in⠀In-Patient,⠀ER, and⠀Out-Patient⠀setting to⠀deploy the⠀optimal⠀CHW⠀strategy⠀to address⠀both⠀Social⠀Determinants⠀of Health⠀and access⠀to⠀care
• Identify⠀key⠀Community⠀Agencies⠀or Church⠀Leaders or⠀Physicians⠀to partner⠀with; to⠀ensure the⠀underserve⠀population's⠀needs are⠀being⠀addressed.
• Provides⠀direction⠀and⠀support⠀for local⠀hospital/community⠀advisory⠀council⠀engagement⠀in the⠀community⠀and social⠀impact⠀agenda of⠀the⠀enterprise.
• Performs⠀human⠀resources⠀responsibilities⠀for staff,⠀including⠀but not⠀limited to⠀interviewing⠀and⠀selecting⠀new⠀employees,⠀promotions,⠀staff⠀development,⠀performance⠀evaluations,⠀compensation⠀changes,⠀resolution⠀of⠀employee⠀concerns,⠀corrective⠀actions,⠀terminations,⠀and⠀overall⠀employee⠀morale.
• Expected⠀to attend⠀various⠀community⠀events⠀after⠀hours and⠀over the⠀weekend.
• It⠀is not⠀designed⠀to cover⠀or contain⠀a⠀comprehensive⠀listing of⠀activities,⠀duties or⠀responsibilities⠀required⠀of the⠀incumbent.
• This⠀position⠀will work⠀with all⠀our 12⠀Hospitals,⠀FQHC⠀Partners,⠀Medical⠀Group &⠀Community⠀Physician⠀Partners;⠀will be⠀required⠀to travel⠀at various⠀sites as⠀needed.

Qualifications

• Bachelor⠀Science⠀degree in⠀health and⠀human⠀services⠀or related⠀area.
• Leadership⠀Experience⠀Required:⠀At least⠀three⠀years of⠀leadership⠀experience⠀in health⠀care,⠀community⠀relations,⠀community⠀engagement,⠀or⠀community⠀health.
• Must⠀be willing⠀to travel⠀to various⠀locations⠀and⠀markets⠀occasional⠀nights and⠀weekends⠀required.
• Ability⠀to present⠀a positive⠀image of⠀the⠀organization⠀while⠀always⠀maintaining⠀high⠀integrity.
• Ability⠀to develop⠀persuasive⠀arguments⠀and⠀present⠀them⠀coherently⠀and⠀effectively.
• Excellent⠀written⠀and verbal⠀communication⠀skills.
• Proficiency⠀in⠀Microsoft⠀Office⠀-Word,⠀Excel,⠀PowerPoint,⠀Access or⠀similar⠀products.
• Ability⠀to work in⠀a⠀fast-paced⠀environment⠀with⠀established⠀time⠀constraints.
